• The information provided in this application and its attachments is accurate, complete, and true to the best of my knowledge.إن المعلومات الواردة في هذا الطلب ومرفقاته صحيحة وكاملة وحقيقية على حدّ علمي.
• I will conduct the study strictly in accordance with the protocol approved by the Research Ethics Committee, and will not implement any change without prior approval.سأُجري الدراسة بدقة وفق البروتوكول المعتمد من لجنة أخلاقيات البحث، ولن أُنفّذ أي تغيير دون موافقة مسبقة.
• I will obtain informed consent from all participants (or their legal representatives) before any study procedure, and will respect their right to withdraw at any time.سأحصل على الموافقة المستنيرة من جميع المشاركين (أو ممثليهم القانونيين) قبل أي إجراء، وسأحترم حقهم في الانسحاب في أي وقت.
• I will protect the privacy of participants and the confidentiality of their data, and will store and handle data securely.سأحمي خصوصية المشاركين وسرّية بياناتهم، وسأخزّن البيانات وأتعامل معها بشكل آمن.
• I will promptly report to the Committee any adverse events, unexpected outcomes, protocol deviations, or new information affecting the risk–benefit balance.سأُبلغ اللجنة فوراً بأي أحداث ضائرة أو نتائج غير متوقعة أو انحرافات عن البروتوكول أو معلومات جديدة تؤثر في موازنة المخاطر والفوائد.
• I will comply with continuing-review requirements and submit progress and final reports as requested.سألتزم بمتطلبات المراجعة المستمرة وأقدّم التقارير المرحلية والنهائية عند الطلب.
• I confirm that all personnel involved are qualified and adequately trained for their roles in this study.أؤكد أن جميع العاملين في الدراسة مؤهلون ومدرّبون بشكل كافٍ لأداء أدوارهم فيها.
